Zepbound vs Wegovy HD in 2026: What Lilly's New Comparison Data Means for Patients

Lilly's new analysis, presented at EASD on September 29, says no. Because no head-to-head trial of the two doses exists yet, Lilly ran a covariate-adjusted indirect treatment comparison — a statistical method that lines up results from two separate trials, SURMOUNT-1 (Zepbound) and STEP UP (Wegovy HD), after adjusting for differences in age, gender, baseline weight, and baseline A1C between the trial populations.

In the analysis's primary (treatment-regimen) estimate over 72 weeks, Zepbound 15 mg was associated with 4.5% greater body weight loss than Wegovy HD 7.2 mg, and Zepbound 10 mg with 3.2% greater weight loss. People on Zepbound 15 mg also had roughly 3.5 times higher odds of losing at least 20% of their body weight.

For context, in their original trials each drug posted strong absolute results. In SURMOUNT-1, participants without diabetes lost an average of 20.9% of body weight on tirzepatide 15 mg and 19.5% on 10 mg at 72 weeks, with 57% of those on the top dose losing at least 20%. In STEP UP, Wegovy HD produced 20.7% average weight loss at 72 weeks in adults without diabetes. Those top-line numbers look close — which is exactly why the adjusted comparison matters: the trials enrolled different populations, so raw averages are not directly comparable.

This was not a head-to-head trial, and that distinction is more than a technicality. Indirect comparisons are a legitimate research tool, but they depend on statistical adjustment rather than randomization, and they were run by Lilly, which makes the drug that came out ahead.

The analysis itself contains a signal of that uncertainty. Under its secondary (efficacy) estimate — which models what happens if everyone stays on treatment as planned — the differences shrank to 1.8% for the 15 mg dose and 0.7% for the 10 mg dose, and neither was statistically significant. In plain terms: among people who stay on either drug consistently, the gap may be small. The larger advantage showed up in the estimate that reflects real-world patterns, where people sometimes pause or stop treatment.
