The SURMOUNT-1 trial found that patients on 15 mg lost an average of approximately 20.9% of body weight over 72 weeks — a substantial difference from the 5 mg arm. That's why providers generally aim to get patients to the highest tolerated dose rather than stopping at the first effective one.
The 12.5 mg dose functions as a bridge. Some patients find it's their personal ceiling, where the side effects at 15 mg outweigh the incremental benefit. Others tolerate 15 mg well and see their best results there.
There's no clinical evidence that higher doses are always better for every patient. The goal is finding the highest dose you can sustain comfortably over time.
