Thousands are injecting unapproved "reta" bought online. A new JAMA Network Open study of 150,000 Reddit posts found surprising symptoms — increased hunger topped the list. What it means for you.

Overview

Retatrutide — the experimental "triple G" weight loss drug nicknamed "reta" online — is still in Phase 3 trials and is not approved by the FDA. That has not stopped thousands of people from buying gray-market versions online and injecting them at home. Now, for the first time, researchers have put real numbers on that experiment. A new study in JAMA Network Open from the University of Pennsylvania used AI to analyze nearly 150,000 Reddit posts and comments, identifying more than 13,500 people who said they were personally taking unapproved retatrutide — and what they reported does not match what the clinical trials predicted. Here is what the study found, why the differences matter, and what it means if you are tempted to try reta before it is approved.

What the Study Looked At

Researchers at Penn's School of Engineering and Applied Science and the Perelman School of Medicine, with a colleague at Boston Children's Hospital, analyzed nearly 150,000 Reddit posts and comments from roughly 40,000 users discussing retatrutide. Using AI language models, they identified about 14,000 users who described personally taking an unapproved version of the drug — in the underlying analysis, 13,589 users reported current use, with posts running through December 2025.

After setting aside expected effects like weight loss and appetite suppression, 7,823 of those users reported at least one symptom the team could map to standard medical terminology. That makes this the largest look so far at what actually happens to people using gray-market retatrutide — a population no clinical trial, and no formal safety monitoring system, is tracking.

It is worth pausing on that last point. Because retatrutide is not approved, nobody selling "research use only" vials online is required to report side effects to the FDA. These Reddit posts are, in a real sense, the only visible record.

The Surprise: Increased Hunger Topped the List

In retatrutide's Phase 2 clinical trials, the most common side effects were gastrointestinal — nausea above all, which is the familiar story with GLP-1 medications like semaglutide and tirzepatide.

The Reddit data looked different. As ScienceDaily reported, the most commonly reported symptom among gray-market users was increased appetite — the opposite of what an appetite-suppressing drug is supposed to do. Fatigue, increased energy, nausea, food cravings, insomnia, and elevated heart rate rounded out the most frequent reports.

Compared with the team's earlier analysis of more than 400,000 Reddit posts about semaglutide and tirzepatide, retatrutide users reported proportionally more appetite changes, sleep disturbances, elevated heart rate, and muscle-related symptoms — and fewer digestive complaints.

Why Would an Appetite Suppressant Make People Hungrier?

The honest answer: nobody knows yet, and the study's authors are careful to say the data cannot prove retatrutide causes any of these symptoms. But the possibilities are instructive, because most of them point back at the gray market itself.

The vials people buy online are not verified. They may be underdosed, degraded, contaminated, or contain something other than retatrutide entirely — problems the FDA has repeatedly documented with unregulated GLP-1 products, including the ingredient-factory failures behind some imported material. A product that is weaker than labeled, or that a user cycles on and off, could plausibly leave people hungrier. Users may also be stacking multiple experimental peptides at once, as the researchers note. Or retatrutide's glucagon-receptor activity — the mechanism that distinguishes it from today's approved drugs — may genuinely behave differently outside the controlled dosing of a trial.

Whichever explanation turns out to be right, none of them is reassuring for someone injecting an unverified vial at home. And the elevated heart rate reports deserve particular respect: retatrutide's trials have noted heart-rate increases, and that is exactly the kind of effect you want a prescriber monitoring.

The Bigger Picture: Gray-Market Reta Is Still a Gamble

This study lands on top of warnings that already existed. The FDA has received adverse event reports tied to unapproved retatrutide — including two deaths reported earlier this fall — and no gray-market seller can legally supply the drug for human use. "Research use only" labeling is a legal fig leaf, not a safety standard.

None of this means retatrutide itself is a bad drug. Its Phase 3 program has produced some of the most striking weight loss results ever recorded, and if those results hold, FDA review is the next step. The problem is not the molecule; it is the supply chain people are using to get it early — no sterility assurance, no dose verification, no screening for contraindications, and no clinician watching for the exact symptoms this study surfaced.

What to Do Instead If You Want Results Now

If the appeal of reta is simply "I want effective medical weight loss," you do not need to wait for it — or gamble on it. FDA-approved GLP-1 medications are available today through licensed providers, with verified dosing and medical supervision. Through JoeyMed's weight loss program, a licensed provider can evaluate whether treatment such as tirzepatide fits your health profile, screen for contraindications like a personal or family history of medullary thyroid cancer or pancreatitis, and set up a titration plan designed around your response.

Real side effects still exist with approved medications — nausea, constipation, and fatigue are common, especially during dose increases — but they are known, monitored, and manageable, often with simple anti-nausea support. That is the difference a regulated product and an actual prescriber make: when something feels off, you have someone to call, and you know what is in the vial.

How to Get Started With JoeyMed

Getting evaluated takes a few minutes online. Start at joeymed.com/weight-loss, complete a confidential health assessment, and a licensed provider will review your history, discuss which medication — if any — is appropriate for you, and ship it from a licensed US pharmacy with ongoing check-ins. Treatment decisions are always made with your provider, results vary from person to person, and medical weight loss works best alongside nutrition, strength training, and sleep. If you have been eyeing gray-market reta, the smarter move is a five-minute conversation about the options that are already approved.
