Medicare's new GLP-1 Bridge covers Wegovy, Zepbound, and Foundayo for a flat $50 a month through 2027. See who qualifies, the BMI criteria, the fine print, and your options if you don't.

Overview

For years, the biggest barrier between older adults and GLP-1 weight loss medication was not eligibility — it was price. That changed on July 1, 2026, when Medicare launched the GLP-1 Bridge, a demonstration program that lets qualifying Part D beneficiaries fill certain GLP-1 prescriptions for a flat $50 a month. According to Newsweek, more than 600,000 Medicare beneficiaries enrolled in the program's first two months, a sign of how much pent-up demand existed among seniors who had been priced out of medications that historically ran over $1,000 a month out of pocket. Here is what the program covers, who qualifies, and what to do if you don't.

What the Medicare GLP-1 Bridge Actually Is

The GLP-1 Bridge is a time-limited demonstration run by the Centers for Medicare & Medicaid Services (CMS), announced in a May 2026 press release and running from July 1, 2026 through December 31, 2027. Rather than changing Part D plans one by one, CMS handles claims processing and pharmacy payments centrally, so your Part D plan does not have to opt in — and, per CMS, any issues go through Medicare directly rather than your plan.

"These treatments are a major medical advancement, but too many seniors are currently unable to access them due to high cost," CMS Administrator Dr. Mehmet Oz said when the program was announced. It is designed as a bridge to a longer-term Part D coverage model (the BALANCE Model), which is why it has a fixed end date.

Which Drugs Are Covered — and What $50 Gets You

Three brand-name GLP-1 medications are covered at the $50 monthly copay, according to AARP's breakdown of the program: Wegovy (semaglutide, in both weekly injections and the newer daily tablets), Zepbound in the KwikPen form (tirzepatide, the same dual-agonist molecule behind JoeyMed's Tirz HD program), and Foundayo (orforglipron), the daily GLP-1 pill approved earlier this year.

The $50 is a flat copay for a monthly supply, which is a dramatic discount from list prices. But there is fine print worth knowing: that $50 does not count toward your Part D deductible, your annual out-of-pocket maximum, or the Medicare Prescription Payment Plan, and it is not reduced further by the Low-Income Subsidy. You are paying $50 in a lane that runs parallel to your normal drug coverage, not inside it.

Who Qualifies for the $50 Copay

Eligibility mirrors the clinical criteria many insurers use, with specific BMI thresholds that must be met when treatment starts. Per AARP, you must be 18 or older, enrolled in an eligible Part D plan (standalone or through Medicare Advantage), and meet one of three bars: a BMI of 35 or higher; a BMI of 30 or higher plus chronic kidney disease (stage 3a or later), heart failure with preserved ejection fraction, or blood pressure that stays high despite two medications; or a BMI of 27 or higher plus prediabetes, a prior heart attack or stroke, or symptomatic peripheral artery disease.

Notably, people with type 2 diabetes, moderate-to-severe sleep apnea, or MASH (fatty liver disease) are excluded from the Bridge — not because they can't get GLP-1s, but because Medicare Part D already covers GLP-1 medications for those diagnoses through the normal benefit. If that is you, your path runs through your existing plan, and it may cost more or less than $50 depending on your coverage.

AARP estimates about 3.8 million Medicare beneficiaries could qualify. Enrollment starts with your prescriber: your provider submits a prior authorization request, and CMS targets a 72-hour turnaround on approvals. You can check whether you might qualify at Medicare.gov/glp1bridge or by calling 1-800-MEDICARE.

What the Numbers Mean for Patients

Six hundred thousand enrollees in two months is one of the fastest uptakes of any Medicare demonstration, and it matters beyond the Medicare population. It signals that GLP-1 pricing is moving down across the board — brand manufacturers have already introduced direct-to-consumer pricing well below the old list prices, and broader coverage tends to pull cash-pay prices down with it.

The medications themselves are unchanged, and so are the responsibilities that come with them. GLP-1s commonly cause nausea, vomiting, diarrhea, or constipation, especially during dose increases — manageable for most people, and JoeyMed offers anti-nausea support alongside treatment for exactly this reason. They are not appropriate for people with a personal or family history of medullary thyroid carcinoma or MEN 2 syndrome, and a history of pancreatitis calls for extra caution. Whether the copay is $50 or not, the decision to start belongs in a screening conversation with a licensed provider.

If You Don't Qualify — or Aren't on Medicare

Most adults considering GLP-1 treatment are not Medicare beneficiaries, and the Bridge does nothing directly for them. If you are under 65, or on Medicare but outside the BMI and comorbidity criteria, you still have real options: manufacturer direct-purchase programs, insurance coverage where your plan includes weight management, and telehealth-based programs that pair prescription treatment with clinical follow-up. Our guides to what Wegovy actually costs in 2026 and who qualifies for GLP-1 treatment walk through those numbers in detail.
